Medicare Care PlansAccepted Here
Use your GP-issued Chronic Disease Management referral for chiropractic care at either of our Melbourne clinics.
Our Solution for Chronic Pain Without the Ongoing Cost
Consistent chiropractic care helps chronic conditions, but cost often gets in the way. A Medicare CDM Care Plan solves this: a GP referral for up to 5 rebated sessions a year, accepted at both our clinics.
Just bring your referral to your first visit. Family-owned for over 20 years, we coordinate your care directly with your GP.

How the Care Plan Process Works
Step 1
Obtain a Chronic Disease Management referral from your GP.
Step 2
Call to book your initial appointment and advise reception you have a Care Plan.
Step 3
Bring your referral paperwork to your first consultation.
Step 4
Practitioner conducts a thorough initial assessment and reviews your care goals.
Step 5
Treatment begins and Medicare rebate is processed, gap fee payable at time of visit.
Step 6
Schedule follow-up sessions (up to 5 per calendar year under the plan).
Most Care Plan patients begin treatment at their very first visit once their referral is confirmed, with follow-up sessions scheduled across the calendar year as needed. Care Plan visits are available at our Taylors Lakes and Gladstone Park clinics.

Understanding the Rebate and Gap Fee
A Medicare CDM Care Plan provides a rebate toward each of up to 5 sessions per year, and patients pay a gap fee (the difference between the practice fee and the Medicare rebate) at the time of each visit. Current gap fee amounts are not yet confirmed for publication: [INSERT CONFIRMED GAP FEE AMOUNT]. Please call the clinic to confirm exact costs before your visit.
Generally, Medicare and private health insurance cannot both be claimed on the same visit. Our reception team can help you work out which option (Care Plan or private health extras) suits your situation best, and can walk you through private health fund claiming details.
